How the Program Works
IM4Q uses trained independent monitoring teams (at least 2 people) to interview adults and children aged three and over who are receiving services through ODP, and their families, about the quality of their lives. IM4Q sends interviewers to talk to people who live in licensed community homes. Interviews are also conducted with people who live with their family, life-sharing families, independently, in intermediate care facilities (ICFs/MR), nursing facilities and personal care homes. IM4Q involvement is completely voluntary, and people and families are asked to participate on a random basis. Interviews are conducted in person at a time and place convenient to the person. The interview takes about an hour to complete using a standard survey instrument. Local IM4Q programs enter interview results into a confidential database, and information is shared only when the individuals grant permission.
The County ID/A Program uses the results of interviews for quality improvement purposes, such as making changes to policy and addressing other considerations raised by the individual, family or staff. Reports created from IM4Q interviews are shared with provider agencies, administrative entities/counties, and the Information Sharing and Advisory Committee (ISAC) for the Office of Developmental Programs for purposes of improving peoples’ quality of life.
Individuals receiving services are asked if an IM4Q team can interview them. They can choose whether or not to participate.

IM4Q allows for the identification of "considerations," which are things that the individual wants to improve in their life, which can lead to service and support changes in their Individual Support Plan (ISP).

The information gathered through IM4Q interviews is used by providers to improve their practices regarding quality improvement efforts for the individuals they support, and by ODP to inform policy and practices, and support the Centers for Medicare & Medicaid Services (CMS) Waiver Assurances.

IM4Q is conducted by independent teams, ensuring objectivity and impartiality in the monitoring process.

Data gathered from IM4Q interviews is anonymously aggregated and publicly shared in annual reports.
